Complete the 2018 Pension Payment Confirmation form

It’s that time again! As a retired member (or surviving spouse), you are required to complete the Pension Payment Confirmation form. The form can be found on page 7 of your Annual Statement, with instructions for completing it on page 6.

You must return the completed and signed form to the CAAT Pension Plan by 
May 31, 2018.

Failure to do so may result in the temporary suspension of your pension, until we receive confirmation.

The pension confirmation process is one of the ways we keep your pension safe, by ensuring we are paying the right pension to the right person. As a retired member (or surviving spouse), you are required to complete this form and return it to the Plan.

What you need to do

By following these steps, you will help us keep our records up to date and your pension payments on track.

If you are the pension recipient:

  • Detach page 7 of your Annual Statement along the perforated edge.
  • Complete section A and check the box to confirm we’re paying the right pension to the right person.
  • Sign and date section A.
  • Return the completed form to the CAAT Pension Plan using the postage-paid return envelope included in this package.

If you are unable to complete the form on your own:

If you are represented by the holder of a valid and operative Power of Attorney for Property, that individual is required to:

  • Detach page 7 of the Annual Statement along the perforated edge
  • Complete, sign and date section B.
  • Return the completed form to the CAAT Pension Plan using the postage-paid return envelope included in this package.
  • If you have not already done so, include a copy of the valid and operative Power of Attorney for Property, as well as proof of your identity, with the form.

It’s important that we receive the completed Pension Payment Confirmation form by May 31, 2018; failure to return the form by the deadline may result in the temporary suspension of your pension, until we receive confirmation. This annual process only takes a few minutes, and will help us keep our records up to date.
