CAAT RCA Contributions Remittance Summary - Purchased Service 020-C (Excel ony)
- This form accompanies your monthly contribution remittance on behalf of any members who contribute to the CAAT RCA.
Change of information or beneficiary (Updated April 2012)
- Report a change to member information (e.g. employment status), personal information (e.g. name, marital status), or to change the non-spouse beneficiary designation.
Pension Application
- Report the end of membership for a member who is retiring

Termination Benefit Application
- Report the end of membership for a member who is terminating employment

Death Benefit Application
- Use when an active member dies to report their final data

Pension Membership Enrolment
- Enrol a new full time or other than regular full time employee as a member of the CAAT Pension Plan.
- This form is 2 pages. The member signs page 1 (Member Authorization) and page 2 (Consent for Collection and Use of Personal Information).
Membership waiver
- An OTRFT employee, who is eligible for enrolment in the Plan, declines membership. Keep the signed original in your files.
Leave of absence
- Report a member's unpaid leave of absence. Include the leave cost and deadlines, and whether or not the member chooses to purchase the period of leave.
- Do not use this form to report a Pregnancy/Adoption Parental leave.

- Report a member's pregnancy, adoption or parental leave. Include the leave cost and deadlines, and whether or not the member chooses to purchase the period of leave.
Notice of LTD/WSIB
Notice of WSIB Partial Disability Benefit
- Notify us when an employee starts and subsequently stops WSIB Partial Disability Benefit status.
Deferred/Survivor Pension Claim
- Notify the CAAT Pension Plan that a member is ready to start receiving their deferred pension.
- Notify the CAAT Pension Plan of the death of a retiree, and to report a claim to the survivor pension, if any.
Pensioner Change request
- For pensioners to report a change in personal or banking information directly to the CAAT Plan.
2011 CAAT Support Strike purchase (2012 rates)
Use this Excel spreadsheet to calculate the cost to the member of purchasing the 2011 strike period (for purchases made in 2012). This form must be signed and accompanied by a cheque from the member for the entire amount of the service purchase.
