Pension Membership Enrolment
Use this form to enroll a Member in the CAAT Pension Plan.
This form gives the CAAT Plan the information needed to enroll the Member and track their service and earnings.
NOTE: This form is 2 pages. Once both pages have be completed and signed, please keep copies and submit the originals to the CAAT Pension Plan.

Section A – Member Identification
Include all employment information about the Member.
NOTE: If the Member is OTRFT and working at only one College, the Date of Enrolment has to be at least 2 years after the Date of Hire.
It is important that you collect all the information on concurrent employment:
- Ask the Member if they are currently employed at another College and, if so, their status at the other College. If they are already Full-Time at the other College, they cannot contribute on their earnings from your College.
- If they are OTRFT at the other College, and OTRFT at your College, they make contributions on earnings from both Colleges.
Ask the Member if they have ever been a Member of the CAAT Pension Plan before and if so, the name of their previous College. If they terminated, please indicate the termination option they chose at that time.
When enrolling a new Member, you must ask them if they have any service that is eligible for purchase or any service that can be transferred into the Plan. Most transfer agreements have time limits and costs usually increase if the purchase is delayed.
Section B – Personal Information
Use this section to collect the Member's personal information, including the information about the Member's Spouse, if relevant.
NOTE: The Spouse is the automatic recipient of death benefits. It is not necessary to complete this section in order for the Spouse to receive death benefits.
The Non-Spouse Beneficiary designation is used to name a Beneficiary if is the Member has no Spouse, or, if the Member does have a Spouse, to name someone in addition to the Spouse in the event the Spouse and Member die at the same time.
- The Member can designate anyone they want as Beneficiary.
- The Member must indicate the full name and date of birth of each Beneficiary, as well as the % of the death benefit each Beneficiary should receive. Remember that the total % can not equal more than 100%.
Section C – Member Authorization
Have the Member read, sign and date Section C. Please note that by signing this, they are also confirming that they have read and signed page 2, the Consent for the Collection and Use of Personal Information.
Section D – College Authorization
Please print your name, date and sign the form.

Consent for the Collection and Use of Personal Information
The Member must read and sign the Consent for Collection and Use of Personal Information (page 2) at the time of enrolment.
Both pages must be submitted together in order to enroll the new employee in the Plan.
