This form is 2 pages. The Member must sign page 1 (Member Authorization) and page 2 (Consent for Collection and Use of Personal Information).

Use this form to report the end of membership of a Member who has terminated their employment, retired or died.

This form is 3 pages long. The Member must initial pages 1 and 2, and sign page 3.

Retirement Advance Election (Form 007)

Use this form when an unmarried Member retires and opts to receive pension payments without first reviewing the Retirement Option Document. This form must be submitted with the TRD Pension Claim (Form 006).
To qualify for the Retirement Advance Election, the Member must:

be unmarried
have no former Spouse with a claim to any benefit from the Plan in respect of the Member
be over age 55

Deferred/Survivor Pension Claim (Form 009)

This form is used to notify the CAAT Pension Plan that a Member is ready to start receiving their Deferred Pension.

This form is also used to notify the CAAT Pension Plan of the death of a retiree, and to report a claim to the Survivor Pension, if any.
