Change my address, banking, beneficiary or spousal information with the CAAT Pension plan?

If you've changed your address or bank account, or need to update beneficiary or spousal information, it's important to let us know as quickly as possible. We've created the online Pensioner Change Request form so you can keep us up to date and avoid delays in processing your pension.

The form is convenient and easy to use in Microsoft Word and Adobe Acrobat format. You can fill it in using your computer, then print out the completed form. Or, print out the blank form then fill it in by hand.

To fill in the form on your computer, simply click in the first field and start typing. Use the "tab" keys to move from field to field, or use your mouse to click on a specific location on the form.
To fill in checkboxes, just click your mouse in the selected box.
Once you have completely filled in the form, print it, sign it and send it to the CAAT Plan. We can't accept electronic copies - the form has to be signed by you and sent via mail or fax to:
